🎯 Eligibility for Council-Funded Support

✅ You or the person being referred are eligible if you/they have any of the following:

Autism

A learning disability

A mental health need

Another long-term condition or impairment

An EHCP in place

And struggle with two or more daily tasks, like:

Cooking or shopping 

Washing, dressing, or managing personal hygiene

Leaving the house or getting around safely

Feeling isolated or unable to take part in activities
